DUPLEX ULTRASOUND AND ULTRASOUND ANGIO EXAMINATIONS IN CASES OF CHOROIDEAL MELANOMA TREATED WITH RUTHENIUM SCLERAL PLAQUES

Patients and Methods: During the last three years 56 patients with choroideal melanoma treated with Ruthenium (Ru) scleral plaques were examined with a Diasonics VST Color-Doppler instrument. We measured the indexes of resistance and of pulsatility, as well as systolic, diastolic and average flow velocities in the blood vessels of the orbit and the tumor with a 10-15 MHz transducer and with the duplex ultrasound and ultrasound angio techniques before, during, 1 day after, and finally 1, 3 and 6 months following Ru irradiation.
Results: We demonstrated that the irradiation raised the resistance index in the tumor from 0,52-0,68 to 0,77-0,92 as well as in the orbital vessels within few days, while changes in other parameters were not significant (p=0.001). Reduction of the blood and oxygen supply may have contributed to the direct radiation effect on the tumor cells resulting in regression.
Conclusion: By the duplex ultrasound examinations and the measurement of resistance indexes we can get information on the effectiveness of plaque therapy at the very early stage, and can make modifications in the treatment if necessary.
